    3) Method of confirming that the person making the Request for Disclosure, etc. is the person himself/herself or his/her agent

    We will confirm that the person making the request for Disclosure, etc. is the principal person or the representative person himself/herself with the following documents (any one of them).
    In the case of an application by a representative, both the “principal person” and the “representative person” are required.

    c) Basic Resident Register card with photo (within the validity period)
    d) Alien Registration Card (within the validity period)
    e) Certificate of residence (valid for 3 months or less)

    For requests for disclosure by a representative, in addition to a) through f) above, the following documents must be submitted as proof of the representative's identity.

    Documents to verify the qualification of the legal representative (a copy of the family register, an extract from the family register, a certificate from the family court, etc., the original of which was made within 30 days) = 1 copy

    The Company charges a fee of 1,000 yen (excluding tax) to the person or his/her representative for a request for disclosure, etc. The fee can be paid in cash at the time of application, or by postal money order or equivalent stamps if sent by mail.
